Operative words
12.2.1 Each facility shall develop and implement a policy and procedure to ensure 100% of employees, [direct contractors, and support staff are vaccinated]... (A) All employees, direct contractors, and support staff must have received their first dose of the COVID-19 vaccination no later than September 30, 2021. [(B)... second dose] of the COVID-19 vaccination (if applicable) no later than October 31, 2021.... (E) On or after October 31, 2021, each facility shall ensure all newly hired employees, direct [contractors, and support staff are fully vaccinated in] accordance with this Part 12.... 12.4.1 Beginning October 1, 2021, each facility shall report its COVID-19 vaccination rate to the [Department]
