Penalty

Enforced as a control measure under Ind. Code 16-41-2-1 and 410 IAC 1-2.5-79 through the isolation and quarantine orders and Class A misdemeanor of Ind. Code 16-41-9-1.5 (Ex. DUT-IN-001) and, from Aug. 1, 2021, as a directive of the Governor's executive orders (Ex. IN-068; Ind. Code 10-14-3-34 Class B infraction, Ex. DUT-IN-002)

What it required

Laboratories and providers report every confirmed case within 24 hours ('shall be reported'); every positive individual isolate and every unvaccinated close contact quarantine, each 'including exclusion from... employment'; congregate settings including schools, daycare, long-term care, jails and group-living facilities undergo investigation ('shall be performed') and apply the school and congregate-setting measures; investigations of confirmed cases generally 'should be performed' (verb recorded as written).

Retrieval note

Four pages, each footed 'Updated 9.1.21.' Para. (2)(F) requires the investigation to obtain the 'Name and contact information for employer.' Para. (3)(F)(a) carries the K-12 classroom exception to the close-contact definition where distancing and masking standards are met. This version, together with Ex. IN-092 (July 19, 2021), shows the isolation-and-quarantine-with-exclusion-from-employment directive in force on every day of 2021 Q3. Later versions (Sept. 7-8, 2021; 'v.8' Feb. 22, 2022, which changed 'shall isolate' to 'should isolate' with a five-day period) are outside the period and not saved.
