Visitation: still generally prohibited... Phase 3... Visitation allowed with screening and additional precautions... Criteria for relaxing restrictions: no new nursing home onset of COVID-19 cases for 28 days... adequate staffing... access to adequate testing... baseline test of all residents [and] weekly testing of all staff.
