Operative words
1. Unless otherwise provided in this chapter, a person who knowingly violates any provision of this chapter, or of the rules of the department or a local board, or any lawful order, written or oral, of the department or board, or of their officers or authorized agents, is guilty of a simple misdemeanor.
Penalty
simple misdemeanor; laboratories repeatedly failing to report: civil penalty up to $1,000 per occurrence (§ 139A.25(3)).

Made knowing violation of chapter 139A, of the Department's or a local board's communicable-disease rules (including 641 IAC ch. 1), or of any lawful isolation, quarantine or area-quarantine order a crime.
