Operative words
1. Self-Evaluation. All staff, service providers, vendors, or contractors, shall self-evaluate for mild to moderate symptoms listed in Attachment B prior to entering Licensed Facilities or Other Agencies, listed in Attachment A, or providing services in the community. Individuals that are experiencing these symptoms shall stay at home... 2. Temperature screening. All staff, visitors, service providers, vendors, contractors, or any other individual entering Licensed Facilities or Other Agencies listed in Attachment A shall submit to a temperature screening... No individual shall progress beyond the temperature screening area, or provide services in the community, for a Licensed Facility or Other Agency... if a temperature screening of the individual is 100.0 F or 37.8 C or greater... 3. Face Coverings and Respirators. All individuals shall wear face coverings... at all times while in the facility. Healthcare professionals (HCP)... shall wear an N-95 respirator, when available, while caring for patients.
